RISE Transition is a non-clinical, donation-supported program providing peer mentorship and resource navigation. It does not provide therapy, diagnosis, or medical treatment.

Who RISE Transition Is For

RISE Transition serves young people ages 16–21 who are aging out of, or have recently aged out of, the foster care system in Clark County. That includes youth still in care who are preparing for the transition, as well as those who have already aged out and are navigating independence without much of a support structure.
