  1. Reach — Intake & Assessment. A peer navigator sits down with you to understand your full situation — housing, documents, income, immediate needs — and builds a personalized starting point.
  2. Ground — Stabilization Planning. Together, you map out a realistic plan across the two service tracks (typically housing/document stability and wellness/connection), with concrete next steps.
  3. Rise — Active Navigation. This is the working phase — document recovery, housing applications, benefits enrollment, and peer support circles, depending on what your plan calls for.
  4. Heal — Stability Check-Ins. Regular touchpoints with your navigator to track progress, adjust the plan, and troubleshoot anything that's stalled.
  5. Thrive — 90-Day Follow-Up. RISE Stability doesn't end the moment your immediate crisis is resolved. A structured 90-day follow-up checks that stability is actually holding.
