RHTWG provides non-clinical peer support and application navigation. We do not determine eligibility or issue benefits directly — that's handled by the state agency. We help you complete and submit the application accurately.

What Benefits Enrollment Support Looks Like

  1. A trained peer Community Health Worker sits with you. Not a call center, not a stack of forms mailed to you — an actual person walking through it with you.
  2. You gather documents together. Your CHW helps you figure out what's needed — ID, proof of income, proof of residency — and how to get anything you're missing.
  3. You complete the application together. Step by step, with someone available to answer questions as they come up.
  4. Submission and follow-up. Your navigator helps you submit and can support you through any follow-up requests from the state agency.

What It Costs
